History of Service
Derbyshire Leaving Care Service is a project commissioned to provide statutory services to care leavers on behalf of the local authority.
Objectives of Service
Statutory support services to care leavers in accordance with legislation and statutory guidance.
Initial Specific Responsibilities
Leaving and after care support duties.
We are looking for a skilled, caring and committed individual to join our Leaving Care Service. This opportunity will enable a successful applicant to join a busy and experienced staff team to undertake work with care leavers to support and advise them in their transition from care to leaving care.
The post would suit individuals with a background/ experience in working in the social care settings/Youth Work/ work with young people. Experience of working within leaving care field is desirable.
The project provides a statutory service, commissioned by Derbyshire County Council. The primary role of the service is to support and guide and ensure practical assistance for young people leaving care into independence between the ages of 16 and 25 years. This is within the remit of the Children (Leaving Care) Act 2000.
Knowledge or experience of the challenges facing care leavers and key area¿s that impact on their progress (education, employment and training, housing, health, managing finance and independence skills) is essential.
The post will be situated in our Heanor base with an expectation for travel to meet role requirement within Derbyshire to where young people are living.
There will also be some requirement to travel for work from the Swadlincote base.
At times travel is also required to attend meetings and support young people if they live outside of Derbyshire.
There is also a requirement for the successful applicant to be part of a county-wide on call telephone support system during evenings, weekends and bank holidays. This will be approximately once a month, for the duration of the service being offered and will attract an additional on call payment.
